Health Homes is a program created by the NYS Department of Health, to provide individual assistance to Medicaid recipients with serious or chronic health problems. It is NOT home health care or a home health aide program. A Health Homes care manager can assist eligible clients with a wide variety of issues—from helping you find a primary care doctor or specialist and coordinating your care, to helping you find safe, affordable housing or applying for benefits. You do not need to receive other services at Delphi—Health Homes care managers work with patients from every health care provider, and can continue to provide services as long as the patient remains eligible for Medicaid.
Health Home care managers are health care professionals provided at no charge to the patient—the cost of services is covered by Medicaid. Delphi Care Services is a part of both Monroe County’s Health Home agencies: the Greater Rochester health Homes Network (GRHHN) and Health Homes of Upstate NY (HHUNY).
